What to Expect from Hospice Services

Hospice care begins with an assessment by a nurse case manager, who works with you, the treating physician and our hospice medical director to create a plan of care. The nurse case manager coordinates care with a team of professionals including home health aides, social workers or counselors, spiritual counselors, and volunteers. This team works together to help the patient end their journey with dignity and comfort and help their family accept the loss of a loved one. Care is evaluated on an ongoing basis and services such as pain management and counseling are provided as needed. When a loved one dies, we offer bereavement support services for up to 13 months.
